Qualifications
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- One of the following: at least one year of experience in sterile IV admixture procedures; knowledge of non-sterile compounding, immunizations and MTM; ASHP residency; APPE; or equivalent experience.
Responsibilities
- Determines the suitability of individual patients for home care.
- Designs, implements, monitors and evaluates patient-specific pharmacy care plans for home care patients.
- Provides medication-use education to staff, patients and caregivers.
- Prepares and dispenses medications following policy; ensures proper labeling and DEA compliance.
- Maintains inventory and equipment; adheres to safety, quality, and regulatory standards.
