Job summary

Option Care Health is seeking a part-time Clinical Pharmacist to provide safe and effective pharmacy services to home care patients in the奈 Virginia area. This entry-level role collaborates with nursing to educate patients, monitor progress, and ensure regulatory compliance.

The position requires an active PharmD and state licensure, experience with IV admixture or related residency/advanced practice, and strong customer service skills.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license in the State.
• One of the following: sterile IV admixture experience or relatedResidency/Advanced Pract. Exp.

Responsibilities

• Assess patient suitability for home care and collect necessary information.
• Design and monitor patient-specific pharmacy care plans for home care.
• Provide medication-use education to staff, patients and caregivers.
• Ensure continuity of pharmaceutical care across home care and other settings.
• Maintain compliance with regulatory, safety, and quality standards.
