Job summary

Option Care Health is seeking an experienced Clinical Transition Specialist to educate patients and their families about at-home infusion services. You will build relationships with referrals, discharge planners and hospital staff to drive patient placement at alternative sites.

You will collaborate with Account Managers to grow referral rates, ensure timely discharges, and coordinate care transitions while maintaining patient confidentiality and adherence to ACHC/URAC standards.

Qualifications

• Licensed RN or licensed Pharmacist in the state of practice.
• Minimum 2 years experience in healthcare.
• Experience building relationships with internal/external stakeholders.
• Knowledge of ACHC/URAC standards and healthcare regulations.
• Proficient PC skills: Word, Excel, PowerPoint.

Responsibilities

• Maintain relationships with referral sources to grow sales and patient starts.
• Assess patients selected by hospitals to ensure viability for home/ATS care.
• Educate patients and families about services and products.
• Coordinate with internal and external partners to ensure seamless patient transitions.
• Partner with Account Manager to create and execute area business plans.
