Job summary

St. Anthony's Hospice in Kentucky seeks a skilled Registered Nurse to provide and oversee mission-centered hospice care in home and long-term care settings.

The RN will plan, document, and coordinate patient care from admission to discharge, collaborating with an interdisciplinary team to meet medical, emotional, and social needs. The role emphasizes compassionate, patient- and caregiver-centered support, with focus on pain management, safety, and education of care partners within Medicare COP

Qualifications

• Must hold a current license as a Registered Nurse in Kentucky.
• Must have a minimum of two years of clinical nursing experience, preferably in hospice or geriatric settings.
• Must be familiar with computers including internet navigation and email use.
• Must demonstrate commitment to St. Anthony’s mission-centered care.
• Must meet all legal and regulatory requirements of Federal and State governments, Medicare Hospice COP and JCAHO.

Responsibilities

• Provides and oversees mission-centered nursing care from admission to discharge.
• Plans, documents, and coordinates individualized care.
• Works with interdisciplinary team to meet physical, emotional, and social needs.
• Conducts weekly patient assessments and updates the Plan of Care as needed.
• Administers medications and monitors therapy per physician orders.
• Maintains accurate clinical documentation and post-discharge procedures.
• Instructs patient/caregiver on safety, treatments, and disease process.
• Leads and supports LPNs and HCAs to maximize patient care.
