Job summary

St. Anthony’s is seeking a compassionate Registered Nurse to join the hospice team as the Home Hospice Case Manager.

The role coordinates, plans and documents care for hospice patients from admission through discharge, developing individualized care plans in collaboration with physicians and the interdisciplinary team. The RN Case Manager provides skilled nursing care, symptom management and education, while maintaining regulatory compliance and a patient-first philosophy throughout the hospice

Qualifications

• Must be licensed as a Registered Nurse in Kentucky.
• Experience in hospice, home health or palliative care preferred.
• Ability to work with interdisciplinary teams and families.

Responsibilities

• Coordinate, plan and document care for hospice patients from admission through discharge.
• Develop and manage an individualized Plan of Care with physician orders.
• Provide skilled nursing care, symptom management and education.
• Communicate with patients, families and the care team regarding changes in condition.
• Ensure compliance with Medicare Hospice Conditions of Participation.
