St. Anthony’s Hospice seeks a Registered Nurse for Home Hospice care in Henderson, KY.

The role covers planning, coordinating and documenting patient care from admission to discharge, delivering mission-centered nursing support in homes or contracted facilities. Responsibilities include conducting patient assessments, providing hospice-level care per plans, managing symptoms, educating patients and caregivers, and collaborating with physicians and the interdisciplinary team.

Qualifications

• Must hold a current license as a Registered Nurse in Kentucky.
• Minimum of two (2) years of clinical nursing experience, preferably in hospice or related settings.
• Familiar with the use of computers including internet navigation and email use.
• Commitment to the mission-centered care approach.
• Meets all legal and regulatory requirements of Federal and State governments and Medicare Hospice Conditions of Participation.

Responsibilities

• Accurately performs and documents patient assessments per Medicare guidelines and organizational policies.
• Provides hospice-level nursing care following individualized plans of care.
• Responds to physical, mental, social and emotional needs of terminally ill patients and caregivers.
• Maintains an environment promoting well-being for patient, caregiver, and staff.
• Makes weekly routine visits to assess status and update the Plan of Care as needed.
• Administers medications as ordered and monitors effectiveness; documents outcomes.
• Provides patient safety instruction and explains medications and procedures to patients and caregivers.
