Job summary
MercyOne is seeking a Per diem Hospice Nurse Practitioner to deliver one-to-one, compassionate home-based care under medical supervision. The NP will assess hospice patients, collaborate with an interdisciplinary team, and ensure care meets regulatory standards.
Qualified candidates hold an Iowa NP license, DEA registration, and a master's in nursing, with experience in palliative/hospice care and strong communication abilities.
Qualifications
- Nurse Practitioner licensure in Iowa and current DEA registration.
- Masters degree in Nursing from an accredited program.
- Active, unrestricted NP status with hospital/clinical privileges.
- Ability to participate in Medicare and Medicaid programs.
- Strong communication and interpersonal skills; compassionate care.
Responsibilities
- Provide face-to-face assessments of hospice patients at home and in various settings.
- Collaborate with Interdisciplinary Team to determine hospice eligibility and pain management.
- Maintain high standards of integrity and regulatory compliance.
- Serve as liaison with hospice referral sources and provide consultations when needed.
- Ensure all patient care aligns with professional standards and practice regulations.
