An established industry player in healthcare is seeking a dedicated Physical Therapist to provide essential patient care in a home health setting. This role involves developing and implementing rehabilitation plans, conducting assessments, and ensuring thorough documentation of patient progress. The PT would be on call from Friday at 4:30 pm until Monday at 8:00 am (eastern). The PT would be responsible to cover any as-needed visits that arise, as well as scheduled weekend visits including admission visits. Compensation will include an on-call stipend PLUS pay per visit for each visit completed. The Therapist will be responsible for assessing patient health problems and needs, developing and implementing Therapist care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required. The PT would be covering the areas of Greenwich, Stratford, New Haven, and surrounding areas.
