Job summary

Your Health is hiring a Transitional Care Professional to coordinate care for patients transitioning from hospitals or facilities to home in the Upstate SC area, including Greenville, Laurens, and Clinton. You will perform home and facility visits and ensure seamless transitions with the care team.

The role requires reliable transportation and daily travel, full-time employment, and 8-hour shifts from 8am to 5pm, Monday through Friday.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred.
• Certifications or licenses must be in good standing.
• Minimum three years clinical experience preferred.

Responsibilities

• Visit referrals and established patients within assigned facilities.
• Build trust with patients to aid informed decisions during facility stay.
• Coordinate care with the care team and providers for seamless transitions home.
• Arrange services like home health, outpatient rehab, and follow-up care.
• Schedule visits with primary care, virtual providers, specialists, pharmacists, social workers.
• Facilitate communication among healthcare professionals involved in care.
• Support patient and family adherence to the care plan.
• Communicate with patients via phone, email, portal, or in person to gather information.
• Provide compassionate customer service and promptly address concerns.
• Perform general administrative tasks and support staff as needed.
• Use company software to enhance patient care and staff efficiency.
• Be available during normal work hours and travel between offices.
