Job summary

Aveanna Healthcare is seeking a Home Health Scheduler to provide high-level customer service from intake to discharge in our Home Health office. The role includes confirming patient information, scheduling discipline visits, and coordinating with care teams to ensure smooth patient flow.

The ideal candidate has a minimum of 2 years of related experience, strong MS Office skills, and the ability to manage multiple tasks in a fast-paced environment while maintaining professionalism and compliance

Qualifications

• High school diploma or GED required; LPN preferred.
• Minimum of 2 years related experience.
• Experience with HCHB a plus.
• Proficient in Microsoft Outlook, Word and Excel.
• Proven ability to manage in a high call volume environment with accuracy and efficiency.
• Demonstrated ability to prioritize multiple tasks to meet deadlines; collaborate with other departments.

Responsibilities

• Contacts patients to advise them of the order placed and confirm demographic information; communicate financial responsibilities.

Overview

Position Overview: The Home Health Scheduler Full Time is responsible for providing a high level of customer service to our patients and their caregivers from intake to discharge for our Home Health office.

Essential Job Functions May Include

• Contacts patients to: advise them of the order placed on their behalf and to confirm all patient demographic information; to communicate any financial responsibilities and/or additional information necessary for processing the order received
• Scheduling Discipline visits and collaborating with staff providing patient care
• Communicating with community Healthcare providers to assist in patient care
• Demonstrates professional etiquette and courtesy when interfacing with customers
• Complies with and adheres to all regulatory compliance areas, policies and procedures
