Job summary

Precise Home Care seeks a licensed Speech-Language Pathologist (SLP) to provide home-based therapy under physician direction. You will assess, diagnose, and treat clients with speech or swallowing disorders, develop care plans, and document progress for an interdisciplinary team.

The role requires CCC eligibility or certification, active licensure, CPR, a valid driver’s license, and at least one year of healthcare experience. Background screening and drug testing are part of the process.

Qualifications

• CCC in Speech-Language Pathology or Audiology and pursuing required supervised experience
• Licensed to practice Speech-Language Pathology
• Minimum 1 year healthcare facility experience recommended
• Current CPR certification, as applicable

Responsibilities

• Performs speech therapy assessments, diagnostic tests, and treatments for home care clients as per Plan of Care
• Completes assessments and OASIS data where applicable within required timelines
• Develops and updates speech therapy plans to improve client function
• Documents findings, plans, and outcomes in the clinical record and shares with physician and nursing staff
• Supervises home health aides in speech-language therapy activities and participates in team conferences
