Home Health Speech-Language Pathologist (SLP)

Provide personalized, home-based speech therapy to patients with communication, swallowing, or cognitive challenges resulting from illness or injury. Work collaboratively with a multidisciplinary home health team to assess, document progress, and achieve patient goals. Conduct evaluations, implement therapy plans, and educate patients and families on safety, self-care, and therapy techniques.
