Job summary

Human Touch Home Health is seeking a compassionate Speech-Language Pathologist (SLP) for PRN home health opportunities across Fairfax County, including Fairfax, Centreville, Chantilly, Reston, Herndon, Vienna, McLean, Springfield, and surrounding communities.

Choose your cities and availability to build a caseload that fits your schedule. Mileage reimbursement and one-on-one patient care are part of the role, with strong clinical leadership and growth opportunities.

Qualifications

• Active Virginia Speech-Language Pathologist License.
• Graduate Degree in Speech-Language Pathology from an accredited program.
• Current ASHA Certification (CCC-SLP) preferred.
• Valid Driver's License and reliable transportation.
• Strong clinical and communication skills.
• Ability to work independently in patients' homes.
• Home Health Experience preferred, but not required.
• WellSky experience is a plus.

Responsibilities

• Complete speech-language pathology evaluations in patients' homes.
• Develop individualized treatment plans.
• Treat speech, language, communication, cognitive, and swallowing disorders.
• Provide dysphagia treatment and swallowing education as appropriate.
• Help patients improve functional communication and cognitive skills.
• Educate patients, families, and caregivers.
• Monitor progress and adjust treatment plans as needed.
• Coordinate care with the interdisciplinary team.
• Complete reassessments and discharge planning.
• Maintain accurate and timely documentation using WellSky.
