Job summary

Human Touch Home Health in Washington, DC is seeking a compassionate Speech-Language Pathologist (SLP) for PRN home health opportunities across the DC area. You will conduct in-home evaluations and develop individualized treatment plans to support communication, cognition, and swallowing.

This role offers flexible scheduling, one-on-one patient care, and the chance to build your caseload around your availability while making a meaningful difference in patients' daily lives.

Qualifications

• Active DC SLP license required.
• Master's degree in Speech-Language Pathology from an accredited program.
• Strong clinical assessment and communication skills.

Responsibilities

• Complete speech therapy evaluations in patients' homes.
• Develop individualized treatment plans.
• Evaluate and treat speech, language, cognitive, communication, and swallowing disorders.
• Provide interventions to improve functional communication and independence.
• Educate patients and caregivers on treatment strategies and safety.
• Monitor progress and adjust plans as needed.
• Coordinate care with physicians, nurses, therapists, and team members.
• Complete progress assessments and discharge planning.
• Complete accurate documentation using WellSky.

What You’ll Do

• Complete speech therapy evaluations in patients' homes
• Develop individualized treatment plans
