• 2+ years of experience as a Speech Therapist, preferably in home health.

Responsibilities

• Conduct comprehensive assessments for speech, language, and swallowing.

• Educate patients and caregivers on at-home exercises.
• Educate patients and caregivers with clear guidance and techniques for at‑home exercises
• Track and adjust therapy plans based on patient progress, ensuring optimal outcomes
• Collaborate with an interdisciplinary care team (nurses, physicians, PTs, OTs, and more) to deliver coordinated care
• Maintain accurate, timely documentation to meet clinical and compliance standards

Position Overview

We are seeking a Full-Time Speech Therapist (SLP) to join our dynamic and supportive team. This role offers flexibility, autonomy, and the chance to provide truly personalized care. What You’ll Do

• Conduct comprehensive speech, language, cognitive-communication, and swallowing assessments
• Design and implement individualized treatment plans to improve communication, swallowing, and cognitive skills
• Provide hands‑on therapy and practical interventions to promote progress and independence
• Educate patients and caregivers with clear guidance and techniques for at‑home exercises
• Track and adjust therapy plans based on patient progress, ensuring optimal outcomes
• Collaborate with an interdisciplinary care team (nurses, physicians, PTs, OTs, and more) to deliver coordinated care
• Maintain accurate, timely documentation to meet clinical and compliance standards

What We’re Looking For

• Master’s degree in Speech‑Language Pathology from an accredited program
• Active Virginia SLP license and CPR certification
• 2+ years of experience as a Speech Therapist (home health experience preferred but not required)
• Strong clinical judgment and evidence‑based practice skills
• Excellent communication and collaboration abilities
• Compassionate, patient‑first approach
• Experience with Home Care Home Base (HCHB) is a plus
