Job summary

Human Touch Home Health is seeking a compassionate Speech-Language Pathologist (SLP) for PRN home health opportunities in Fredericksburg, Stafford, Spotsylvania and nearby communities. You choose your territory, set availability, and build a caseload that fits your schedule.

Responsibilities include home evaluations, plans of care, dysphagia treatment, and caregiver education with WellSky documentation. Virginia license required; ASHA CCC-SLP preferred.

Qualifications

• Active Virginia SLP license is required.
• Graduate degree in Speech-Language Pathology from an accredited program.
• ASHA CCC-SLP certification is preferred.
• Valid driver's license and reliable transportation.
• Strong clinical and communication skills; ability to work independently in patients' homes.
• Home health experience preferred but not required; WellSky experience is a plus.

Responsibilities

• Perform home-based speech-language pathology evaluations.
• Develop individualized plans of care.
• Treat speech, language, cognitive and swallowing disorders.
• Provide dysphagia treatment and education as appropriate.
• Educate patients, families and caregivers; monitor progress and adjust plans.
• Complete reassessments and discharge planning; document using WellSky.
• Coordinate care with the interdisciplinary team.

Patient opportunities may be available in Fredericksburg, Stafford, Falmouth, Spotsylvania, Massaponax, Spotsylvania Courthouse, Garrisonville, Aquia Harbour, and surrounding areas.

What You'll Do

• Complete speech-language pathology evaluations in patients' homes

• Develop individualized plans of care

• Treat speech, language, communication, cognitive, and swallowing disorders

• Provide dysphagia treatment and swallowing education as appropriate

• Help patients improve functional communication and cognitive skills

• Educate patients, families, and caregivers

• Monitor progress and adjust treatment plans as needed

• Complete reassessments and discharge planning

• Coordinate care with the interdisciplinary team

• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Virginia Speech-Language Pathologist License Required

• Graduate Degree in Speech-Language Pathology from an accredited program

• Current ASHA Certification (CCC-SLP) Preferred

• Valid Driver's License and reliable transportation

• Strong clinical and communication skills

• Ability to work independently in patients' homes

• Home Health Experience Preferred, but Not Required

• WellSky Experience is a Plus
