Job summary

CenterWell Home Health, a Humana company, is seeking a Speech-Language Pathologist to provide services in the home setting. You will evaluate and treat patients, develop plans of care, and coordinate with the medical team to improve communication, swallowing, and overall quality of life.

This role requires a license, CPR, and driving ability to reach patient homes. Experience in home health is preferred; pay per visit range is posted.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver\'s license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify and minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
• Observe, record and report changes in patient condition and response to treatment.
