Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to provide in-home care, assessing and treating speech, language, voice and swallowing disorders, and supporting patients to regain independence. You will develop and review the Plan of Treatment and Plan of Care, document progress, and collaborate with families and healthcare staff to coordinate care.

Adherence to ASHA standards, CPR certification, licensed status, and a valid driver’s license are required; Humana benefits

Qualifications

Must hold CCC in Speech-Language Pathology (ASHA) or Audiology.
Minimum of 1 year experience as a speech-language pathologist.
Current and unrestricted license to practice.
Current CPR certification.
A valid driver’s license and reliable transportation required.

Responsibilities

Evaluate and provide speech-language pathology services in patient homes or facilities.
Participate in development and periodic review of the Plan of Treatment and Plan of Care.
Assess and treat disorders of speech, voice, language, hearing and swallowing to improve function.
Administer and interpret diagnostic tests and apply therapeutic interventions.
Monitor patient progress and communicate changes to supervisor and physician.
