Job summary

Humana Inc. in the CenterWell Home Health network is seeking a Home Health Speech Language Pathologist to evaluate and treat patients at their homes or facilities.

You will participate in care planning, perform diagnostic assessments, and provide training on alternative communication methods. The role requires six months of experience as a speech-language pathologist, a valid license and CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction and training to patients in alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company and/or facility.
• Prepare and submit written reports of evaluations, visits, care plans and progress reports.
• Participate in care coordination activities and discharge planning.
• Maintain confidentiality and professional conduct.
