Job summary

Human Touch Home Health in Washington, DC is seeking a compassionate Speech-Language Pathologist (SLP) for PRN home health opportunities throughout the District. You will perform evaluations, develop treatment plans, and support patients in their homes.

The role offers flexible scheduling, one-on-one patient care, and opportunities to build your caseload around your availability. A DC license and master's in SLP are required, with WellSky experience a plus.

Qualifications

• Active Washington, DC Speech-Language Pathologist License Required.
• Master's Degree in Speech-Language Pathology from an accredited program.
• Strong Clinical Assessment and Communication Skills.
• Dependable Transportation to Patient Visits.
• Ability to Work Independently in Patients' Homes.
• Home Health Experience Preferred, but Not Required.
• WellSky Experience is a Plus.

Responsibilities

• Complete speech therapy evaluations in patients' homes
• Develop individualized treatment plans
• Evaluate and treat speech, language, cognitive, communication, and swallowing disorders
• Provide interventions to improve functional communication and independence
• Educate patients and caregivers on treatment strategies and safety
• Monitor patient progress and adjust treatment plans as needed
• Coordinate care with physicians, nurses, therapists, and other team members
• Complete progress assessments and discharge planning
• Complete accurate and timely documentation using WellSky
