CenterWell Home Health is seeking a skilled Speech Language Pathologist to deliver in-home or facility-based speech therapy. You will evaluate patients, develop treatment plans, and collaborate with a multidisciplinary team to improve communication, swallowing, and function.

A valid license and CPR are required, with at least six months of SLP experience. You will travel to patient homes, maintain thorough documentation, and participate in discharge planning.

Competenze

• Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation are required.

Mansioni

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe, record and report changes in the patient’s condition and response to treatment to supervisor.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation and progress reports as required by Company policy.
• Participate in care coordination activities and discharge planning.
