Job summary

Angels Care Home Health is seeking a Speech Language Pathologist (ST) to provide in-home assessment and therapy for patients in Tazewell, VA. The clinician will deliver comprehensive care, develop and adjust care plans, and work with an interdisciplinary team to optimize communication, swallowing, and cognitive outcomes.

The ideal candidate holds a Master’s in Speech-Language Pathology, a current state license, CCC-SLP, and a valid driver’s license, with 2 years in acute care or rehab or home

Qualifications

• Master’s degree in Speech Language Pathology from an accredited college or university.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence (CCC-SLP) from ASHA.
• Valid driver’s license.
• 2 years’ experience in an acute care or rehabilitation setting; home health preferred.
• Reliable transportation with current auto liability insurance.

Responsibilities

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain patient well‑being.
• Collaborates with interdisciplinary team.

Skills
