Job summary

St. Elizabeth Home Care is seeking a skilled Speech Language Pathologist to join the Health at Home team.

The Speech Therapist will assess, diagnose and treat patients in their homes, developing individualized treatment plans and collaborating with families and other healthcare professionals for optimal outcomes. Qualifications

Master's degree in Speech-Language Pathology from an accredited program.
Active SLP license in the state of practice and CCC-SLP certification from ASHA.
Must obtain and maintain CPR/BLS Certification.
One year of Speech Therapy experience in a clinical setting.
Current driver's license.
Comply with federal, state and local laws and regulations.

Responsibilities

Provide therapy to patients in their homes, including techniques to improve speech, language, cognitive-communication, and swallowing skills.
Customize therapy strategies to meet individual patient goals and objectives.
Maintain accurate and timely documentation of patient progress, treatment goals, and outcomes in accordance with regulatory requirements and standards.
Provide training and support for family members to assist with therapeutic exercise and strategies at home.
