Job summary

Angels Care Home Health seeks a Speech Language Pathologist to provide therapy in patients' homes, developing and adjusting plans to improve speech, language, swallowing, and cognition. You’ll work with an interdisciplinary team to deliver high-quality, patient-centered care.

The role requires a Master’s degree in Speech Language Pathology, a current state license, and CCC-SLP. Home health experience and a valid driver’s license are preferred; comprehensive benefits are offered.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited college or university.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence (CCC-SLP) from ASHA.
• Valid driver’s license and reliable transportation.
• 2 years’ experience in an acute care or rehabilitation setting; home health preferred.

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.

The Speech Language Pathologist is a registered professional who provides therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing, and cognitive disorders.

Functions:

• Provides coordinated care to patients in their place of residence.
