Job summary

Angels Care Home Health is seeking a Speech Language Pathologist to provide therapeu­tic services to patients in their homes, evaluating and treating speech, language, swallowing, and cognitive disorders. You’ll work with an interdisciplinary team to plan and implement care plans that restore or maintain health and quality of life.

The role requires a Master’s in Speech-Language Pathology, a current state license, CCC-SLP, a valid driver’s license, and 2 years of experience in acute care or

Qualifications

• Master’s degree in Speech Language Pathology from an accredited college or university.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence from American Speech Language Hearing Association.
• Valid driver’s license.
• 2 years’ experience in an acute care or rehabilitation setting, home health preferred.
• Reliable transportation with current auto liability insurance.

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
