Job summary

Elite Home Health is seeking a Speech-Language Pathologist to deliver home-based therapy across Florida, helping patients improve communication, cognition, swallowing, and independence.

The SLP evaluates, plans, and provides skilled services per physician orders, collaborating with the inter-disciplinary team and ensuring compliant documentation. Travel to patient homes is required; active Florida license and CPR are mandatory.

Qualifications

• Master’s degree in Speech-Language Pathology and active Florida SLP license.
• At least 1 year of clinical speech therapy; home health preferred.
• CCC-SLP preferred; CPR and valid driver's license required.
• Ability to pass background screening and compliance requirements.

Responsibilities

• Evaluate speech, language, cognitive, and swallowing disorders in home visits.
• Develop and update individualized treatment plans and goals.
• Provide skilled therapy and dysphagia management.
• Educate patients and families; coordinate with care team.
• Complete timely documentation and discharge planning.
• Ensure compliance with Medicare/state regulations.
