Job summary

CenterWell Home Health, part of Humana Inc, seeks a Home Health Speech Language Pathologist to evaluate and treat patients at home or in facilities. You will develop care plans, monitor progress, and coordinate with supervisors and physicians to support rehabilitation and communication goals.

The role requires ASHA CCC or equivalent certification, 6 months of SLP experience, a valid license and CPR, plus a valid driver’s license and reliable transportation. Florida background checks apply.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects.
• Observe, record and report changes in the patient's condition and response to treatment to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
