Well Care Health is seeking a dedicated Speech Language Pathologist to provide in-home therapy across the Charlotte, NC area. You will assess, diagnose, and treat communication disorders, collaborate with a care team, educate patients and families, and document progress in the patient's chart.

Qualifications

• Master's degree in speech-language pathology from an accredited program.
• Licensure in NC or SC; CPR certification; CCC from ASHA or CFY eligibility.
• Experience in home health or medical settings preferred.
• Excellent communication and collaboration skills.

Responsabilités

• Assess and treat patients with speech, language, and swallowing disorders in home settings.
• Develop individualized treatment plans and document progress.
• Educate patients and families on management and carryover techniques.
• Collaborate with the clinical team to ensure high‑quality care.
