Job summary

Well Care Health is seeking a compassionate Speech Language Pathologist to provide in-home care across developmental stages from neonates to geriatrics. You will assess, treat, educate families, and collaborate with a clinical team to ensure high-quality patient care.

The role requires a Master's in Speech-Language Pathology, state licensure (NC/SC), and ASHA CCC or CFY eligibility, with a typical 40-hour workweek and opportunities to participate in program development and performance

Qualifications

• Masters degree in speech-language pathology.
• Licensure in the state where providing care (NC/SC); CCC from ASHA or eligibility to begin CFY year.

Responsibilities

• Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments.
• Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans.
• Educate patients and families on health management, medications, and self-care techniques.
• Collaborate with the healthcare team to ensure the highest quality care for each patient.
• Works under the supervision of a Clinical Team Leader.
• Provides individualized patient care for patients in all developmental stages throughout the life span including: Neonates - 0-28 days, Infant - 1-12 months, Child - 1-12 years, Adolescent - 13-17 years, Adult - 18-72 years, Geriatric - 72 + years.
