Job summary

Well Care Health in North Carolina is seeking a compassionate Speech Language Pathologist to join our in-home care team. You will assess, treat, and educate adults and geriatric patients in Duplin County, delivering personalized plans in patients' homes.

Responsibilities include monitoring patient progress, documenting care, and collaborating with a clinical team leader to ensure high-quality services. Licensure in NC/SC and CCC or CFY eligibility are required.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• State licensure in NC or SC with CCC from ASHA or CFY eligibility.
• Experience with clinical practicums in medical settings preferred.
• Excellent communication and documentation skills.

Responsibilities

• Assess and plan patient care across the adult and geriatric population in Duplin County.
• Deliver in-home speech-language pathology services and document care.
• Educate patients and families on communication strategies and self-care.
• Collaborate with the clinical team to develop and adjust care plans.
• Maintain schedules and adapt to patient needs and agency policies.
