Job summary

Compassus is seeking a Home Health Speech Language Pathologist to deliver patient-centered therapy in the home setting across Florida. The role includes comprehensive assessments, Plan of Care development, and collaboration with physicians and the care team to ensure high-quality outcomes.

The position requires a valid SLP license, at least one year of home health experience, and familiarity with adult and geriatric populations.

Qualifications

• Bachelor's degree in Speech Language Pathology required.
• Master's degree in Speech Language Pathology preferred.
• Minimum of one year of experience in a home health setting.
• Experience working with adult, geriatric patients preferred.
• Active and unencumbered Speech Language Pathologist license required in state(s) of employment.

Responsibilities

• Provide speech, language, cognitive, and swallowing therapy to home health clients.
• Develop and revise Plan of Care in collaboration with physician and care team.
• Perform initial and ongoing comprehensive assessments (OASIS/Agency).
• Document goals and outcomes and ensure continuity of care.
• Supervise and provide direction to Home Health Aide/UP to ensure quality and continuity of service.
• Educate clients and families about disease processes and therapy plans.
