CenterWell Home Health seeks a Home Health Speech Language Pathologist to evaluate and provide SLP services in patients’ homes or facilities, under the Clinical Manager. You will contribute to the Plan of Treatment and Care, assess disorders of speech, language, voice, swallowing, and document progress for rehabilitation.

Required: CCC‑SLP or Audiology, at least six months of experience, active license and CPR. A valid driver’s license and reliable transportation are required.

Qualifications

• CCC-SLP or Audiology certification required.
• Minimum six months experience as a speech therapist / SLP.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver’s license and transportation required.

Responsabilités

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess disorders of speech, voice, language, hearing and swallowing to rehabilitate.
• Administer and interpret diagnostic tests and apply therapeutic treatments.
• Record changes and report progress to supervisor and physician.
• Provide instruction on alternative communication systems when appropriate.
• Maintain documentation and submit reports as required by policy.
• Participate in care coordination and discharge planning.
