Job summary

Remarkable is seeking a Speech-Language Pathologist to deliver in-home care in Texas. You will assess, diagnose, and treat speech, language, cognitive, and swallowing disorders with an interdisciplinary team. The role emphasizes goal-driven care, family education, and thorough documentation of the Plan of Care.

Travel to patient homes, on-call rotation, and adherence to agency policies are required. Master’s or Doctoral degree in speech-language pathology with state licensure and 2 years of home

Qualifications

Master’s or doctoral degree in Speech-Language Pathology.
2 years of experience in home health.
Appropriate state licensure and CPR certification.
Adult home health experience preferred; HCHB experience a plus.

Responsibilities

Performs comprehensive assessment of speech-language, cognitive, and swallowing disorders.
Establishes measurable short- and long-term goals with time frames.
Provides treatment addressing speech-language, cognitive, and swallowing disorders.
Educates patients, families, and staff about diagnoses, treatment rationale, and plans of care.
Documents therapy visits accurately and coordinates care with the physician.
Participates in IDT meetings and on-call rotation.
Maintains confidentiality and complies with agency policies.
