Job summary

Ascension at Home together with Compassus is seeking a Home Health Speech Language Pathologist - PPV in Wisconsin. The role involves developing therapy plans, conducting patient visits, and coordinating with physicians, families, and caregivers to support speech and language rehabilitation in a home health setting.

The candidate should have a Bachelor's in Speech Language Pathology, with a Master’s preferred, and at least one year of home health experience.

Qualifications

• Bachelor's degree in Speech Language Pathology required.
• Master's degree in Speech Language Pathology preferred.
• Minimum of one year of home health experience may be preferred.

Responsibilities

• Develops speech therapy plans of care in collaboration with referring physician.
• Performs patient visits for assessment and evaluation; completes OASIS assessments.
• Educates patients, families, and team members; documents care and maintains medical records.
• Provides speech-language pathology services per interdisciplinary plan of care; supports communication and swallowing exercises.
