Job summary

Ascension at Home, in partnership with Compassus, seeks a Home Health Speech Language Pathologist - PPV to deliver patient‑centered care in home settings, evaluating and treating speech and swallowing disorders, and coordinating with physicians and families. Licensure as a Speech Language Pathologist is required, with at least one year in home health preferred.

Qualifications

• Bachelor’s degree in Speech Language Pathology is required.
• Master’s degree in Speech Language Pathology is preferred.
• Minimum of one year of home health experience is preferred.

Responsibilities

• Develop speech therapy plans of care after physician consultation.
• Visit patients for assessment and evaluation; complete OASIS assessments.
• Provide education to patients, families, and team members.
• Document care and update patient records in a timely manner.
• Participate in interdisciplinary team meetings and quality improvement.
