Job summary

Compassus is seeking a Home Health Speech Language Pathologist (SLP) to deliver speech and language pathology services in patients' homes. The role involves developing care plans, performing assessments, and educating patients and families as part of an interdisciplinary team.

Requirements include a Bachelor's degree in Speech-Language Pathology, a Master’s preferred, and at least 1 year of home health experience with an active SLP license. Excellent communication and documentation are essential.

Qualifications

• Bachelor's degree in Speech-Language Pathology required.
• Master's degree in Speech-Language Pathology preferred.
• Minimum of one year of home health experience.
• Active SLP license in state of employment.
• Strong written and verbal communication skills.

Responsibilities

• Develops speech therapy plans of care after physician consultation.
• Visits patients for assessment and evaluation and documents in EMR.
• Provides education to team members, patients and families.
• Completes OASIS assessments and coordinates with the physician.
• Participates in interdisciplinary team meetings and discharge planning.
• Maintains accurate medical records and follows infection control procedures.
