Job summary

Ascension at Home together with Compassus seeks a Speech Language Pathologist for home health therapy under physician direction. The role involves collaboration with family, nurses, and the care team to deliver focused assessments and treatments.

You will assess swallowing, communication, and cognitive disorders, develop and update care plans, and educate patients and families while ensuring regulatory compliance and quality care.

Qualifications

• Bachelor's degree in Speech Language Pathology required.
• Master's degree in Speech Language Pathology preferred.
• Minimum of one year of experience in a home health setting.
• Experience working with adult/geriatric patients.
• Active and unencumbered SLP license in state(s) of employment.

Responsibilities

• Provides care with infection control measures for staff and patients.
• Communicates effectively with patient/family and care team.
• Delivers services requiring specialized speech therapy skills.
• Performs initial and ongoing assessment of speech, language, cognition, and swallowing.
• Develops and revises Plan of Care in coordination with physician and team.
• Monitors cases for third-party payor requirements.
• Provides education to client/family about disease processes.
• Performs other duties as assigned.
