Job summary

CenterWell Home Health, part of Humana Inc., is seeking a Home Health Speech Language Pathologist to evaluate, treat and support patients in their homes, focusing on speech, language, voice, swallowing and related communication goals to improve independence and quality of life.

Reporting to the Clinical Manager, you will develop and review plans of treatment, document progress, coordinate with caregivers and physicians, and deliver therapy during in-home visits to help patients regain strength

Qualifications

• Six months minimum experience as a speech-language pathologist.
• Current and unrestricted professional license.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to improve function.
• Administer and interpret diagnostic tests and applications of therapy.
• Document changes in patient condition and response to treatment for supervisor/physician.
• Provide instruction to patients and families on communication strategies and home exercises.
