Job summary

CenterWell Home Health is seeking a Home Health Speech-Language Pathologist to evaluate and treat patients in their homes or facilities. You will develop care plans, document progress, and coordinate with care teams to improve communication and swallowing functions for patients.

The role requires ASHA CCC, a current license, CPR certification, and a valid driver’s license, with preference for home health experience. TB and driver-safety programs may apply.

Qualifications

• ASHA CCC required for licensure and professional practice.
• Minimum six months experience as a speech-language pathologist.
• Current CPR certification and valid driver's license required.

Responsibilities

• Evaluate, direct and provide speech/language pathology services in home or facility settings.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
