Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License and access to dependable transportation
• CPR certification or ability to obtain within 90 days of hire
• Licensed Social Worker in state of residence

• Assists patients, family, physician and care team to understand social, emotional and environmental factors affecting health
• Contributes to development of a comprehensive Plan of Care with the health care team
• Instructs on community resources available to support patients and families
