Job summary

UnitedHealth Group, Caretenders (LHC Group) is seeking a Medical Social Worker to provide medical social services under physician direction and an interdisciplinary team. You will assess social and emotional factors affecting health status and support coping strategies.

Primary responsibilities include helping patients and families understand factors influencing care, contributing to the Plan of Care, linking to community resources, and coordinating bereavement services when needed.

Qualifications

• Master's Degree from CSWE-accredited program.
• Current CPR certification.
• Licensed Social Worker in the state of residence.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• 1+ year of social work experience in a health care setting.

Responsibilities

• Assists patients and families to understand personal, emotional, environmental, and social factors.
• Contributes to an integrated Plan of Care on a daily basis.
• Instructs staff on community resources.
• Able to function as Bereavement Coordinator and plan bereavement services.
