Job summary

UnitedHealth Group is seeking a Social Worker for Assured Home Health, part of LHC Group, to provide medical social services under physician guidance and collaborate with the Interdisciplinary Group to support patients and families in coping with health-related social and emotional factors.

Responsibilities include assessing factors affecting health, contributing to an integrated Plan of Care, and guiding the team to community resources.

Qualifications

• Master's Degree from a CSWE-accredited school of Social Work.
• Current Driver's License, vehicle insurance, and access to dependable vehicle or public transportation.
• Licensed Social Worker in the state of residence (preferred).
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Assists patients and families to understand personal, emotional, environmental, and social factors related to health.
• Contributes to development of a comprehensive, integrated Plan of Care for patients daily.
• Instructs care team on community resources to assist patients as needed.
