Job summary

CenterWell Home Health is seeking a dedicated Medical Social Worker to join our caring interdisciplinary team. Reporting to the Clinical Manager, you will evaluate patients’ emotional and social needs and develop personalized plans of care to support recovery at home.

You will supervise the Social Worker Assistant, coordinate with nurses and other professionals, arrange community resources, and document care plans in accordance with company policies and regulatory requirements.

Qualifications

• Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
• Licensure as a Social Worker in the state of practice, if required by state law.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Assess the patient’s social and emotional state related to illness or injury, care needs and recovery.
• Assess relationships of medical and nursing needs in the home, finances, and community resources.
• Arrange community resources to help resolve issues impacting recovery.
• Communicate with patients, families, and organization staff.
• Provide ongoing assessment of patient and family needs and coping with treatment.
