Job summary

CenterWell Home Health is seeking a Medical Social Worker to join our interdisciplinary care team serving home health patients. You will evaluate and develop personalized plans of care addressing emotional and social needs.

Reporting to the Clinical Manager, you will supervise the Social Worker Assistant, coordinate discharge planning, connect families with community resources, and document care actions in accordance with company policies.

Qualifications

• Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
• Social Worker licensure in the state of practice, if required by state law or regulation.
• A valid driver's license, auto insurance, and reliable transportation.
• Proof of current CPR certification.
• Experience with discharge planning needs, and obtaining community resources.
• Knowledge of medications and their correct administration.
• Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
• Flexibility in work hours and travel locally.
• Maintain current licensure certifications and meet mandatory continuing education requirements.
• Must read, write and speak fluent English.

Responsibilities

• Assess the patient's social and emotional state as it relates to their illness or injury, needs for care and their response to such treatment.
• Assess relationships of the patient's medical and nursing needs in the home setting, financial resources, and community resources.
• Provide actions to obtain available community resources to help resolve issues hindering recovery.
• Communicate with patients, family members, and associates in the organization.
• Provide ongoing assessment of patient and family needs and responses to the treatment process.
• Participate as a member of the interdisciplinary care team in care coordination and support other health team members.
• Evaluate the Social Work Assistant's performance to ensure compliance with standards.
• Maintain and submit documentation including case conferences, visit reports, and progress notes.
