CenterWell Home Health is seeking a Medical Social Worker to participate in interdisciplinary care for home health patients. You will evaluate and develop personalized care plans addressing emotional and social needs, and supervise the Social Worker Assistant when involved in the plan of care.

You will work within company policies, healthcare standards, and applicable regulations while supporting patient recovery and independence.

Qualifications

• Minimum of one year of experience as a social worker in health care, home health, or hospice.
• Current licensure as a Social Worker in the practicing state, if required.
• Valid driver’s license, auto insurance, and reliable transportation.
• CPR certification up to date.

Responsabilités

• Assess the patient’s social and emotional state related to illness and care needs.
• Evaluate relationships of medical and nursing needs in home settings, resources available.
• Identify and connect patients with community resources to aid recovery.
• Communicate with patients, families, and team members to coordinate care.
• Provide ongoing assessment of patient and family needs and coping with the treatment process.
• Participate in interdisciplinary care teams and support other health professionals.
• Evaluate Social Work Assistant performance and ensure services meet standards.
• Maintain and submit required documentation including visits, contacts, and progress notes.
