Job summary

CenterWell Home Health is seeking a Medical Social Worker to join our interdisciplinary care team serving home health patients. You will evaluate and tailor plans of care to address emotional and social needs and direct the Social Work Assistant when involved in the patient’s plan of care.

You will assess social and emotional states, coordinate care with family and staff, and help access community resources to support recovery. Documentation and adherence to company policies are required.

Qualifications

• One year of social work experience in health care, home health, or hospice.
• Social Work licensure in the relevant state.
• Valid driver’s license with reliable transportation.
• CPR certification.
• Experience with discharge planning and community resources.
• Knowledge of medications and their administration.
• Ability to organize tasks, set priorities, and work under pressure.
• Flexible hours and local travel.
• Maintain licensure and continuing education requirements.
• Must read, write, and speak fluent English.

Responsibilities

• Assess the patient’s social and emotional state relative to illness or injury and plan care.
• Assess relationships to medical and nursing needs in the home, finances, and resources.
• Arrange community resources to support recovery.
• Communicate with patients, families, and staff.
• Monitor patient and family needs and coping with treatment.
• Participate in interdisciplinary care planning and act as a resource to other health team members.
• Evaluate Social Work Assistant performance to ensure compliance with standards.
• Maintain and submit required documentation and reports.
