Job summary

BAYADA Home Health Care in Londonderry, NH is seeking an experienced Speech Language Pathologist, SLP for a part-time or per diem role delivering home health visits to adult and geriatric patients in New Hampshire.

Responsibilities include home visits, evaluations, treatment planning, documentation on a touch pad tablet, and collaboration with a multidisciplinary team to promote patient health and independence. NH license and CCC-SLP or equivalent are required or in progress.

Qualifications

• Current license as Speech Language Pathologist in New Hampshire.
• Certificate of Clinical Competence in Speech from ASHA (CCC-SLP) or equivalent qualifications.
• Education/certification requirements as indicated by transcript or diploma, and related supervised experience for certification.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations and provide therapeutic interventions for speech, language, swallowing difficulties.
• Develop and revise treatment plans based on evaluations, physician orders, and goals.
• Document observations and interventions using a state-of-the-art touch pad tablet.
• Educate patients and families on rehabilitative care and activities to promote health and independence.
