Job summary

Concierge Home Care in Sarasota, FL is seeking a Clinical Team Support (Scheduling Coordinator) to help our clinical team deliver timely patient care. You will coordinate patient visits, align clinician schedules with care plans, and manage daily changes in a fast-paced home health environment.

The role requires at least 2 years of scheduling experience, strong organizational and communication skills, and familiarity with WellSky/Kinnser.

Qualifications

• High school diploma required.
• At least 2 years of home health scheduling experience.
• Strong organizational and multitasking skills.
• Ability to work in a fast-paced, team-driven environment.
• Previous use of Kinnser/WellSky in a home health scheduling role.
• Excellent communication and problem-solving abilities.

Responsibilities

• Scheduling patient visits in accordance with physician orders and care plans.
• Coordinating clinician schedules to optimize productivity and coverage.
• Managing daily scheduling changes, call-outs, and urgent visit needs.
• Answering and triaging incoming calls to support clinical operations.
• Tracking medical supplies and supporting RAP/EOE processes.
• Maintaining accurate scheduling and documentation within the EMR.
• Supporting the clinical team with administrative tasks as needed.
