Role Description

The Home Health Scheduler/Intake Coordinator is a full-time, on-site role based in Palm Coast , FL. This role is responsible for coordinating and scheduling home health visits, ensuring that clinicians are assigned appropriately based on patient needs, availability, and location. The coordinator receives and processes new referrals, gathers and verifies patient information, and confirms insurance and eligibility as needed. Daily activities include maintaining accurate schedules in the system, updating appointments and visit changes, and communicating promptly with clinicians, patients, families, and referral sources. The role also involves tracking documentation, following up on missing information, prioritizing urgent requests, and collaborating with the clinical team and management to support efficient operations and high-quality patient care.
