Elara Caring is seeking a Scheduler to support Administrators and Branch Directors in planning patient visits and directing care teams. You will coordinate schedules, streamline workflows, and ensure timely patient care with the clinical team and physicians.

Qualifications

• High school diploma or GED required; Associate degree preferred.
• At least 1 year of practical scheduling experience in home care preferred.
• Familiar with national standards of care, state licensure, and federal conditions of participation.
• Ability to convey information clearly, listen, and respond with appropriate tone.
• Ability to plan ahead, set timeframes, and allocate resources effectively.
• Decision making with incomplete information and escalation when needed.
• Proficiency with Homecare Homebase and Workday; intermediate MS Office skills.
• Bilingual in Spanish and English preferred.

Responsibilities

• Schedule patient visits and coordinate with clinical team managers.
• Ensure timely patient care by aligning staff availability with patient needs.
• Communicate across CTM, direct care staff, physicians, and facility staff.
• Assist with workflow improvements in patient journey.
